New Patients
What you should expect:
I promise quality dentistry in a clean relaxed setting by a courteous and friendly staff. If you are disappointed in any way, you are encouraged to speak to the doctor. I will make every effort to remedy any problem you are having.
I provide a full range of treatments, including General Dentistry, Restorative Dentistry, Orthodontics, Oral Surgery, Cosmetics, Dentures and my favorite, Pediatric Dentistry. We love children.
Although I received advanced training in sedation for the treatment of children, I have discontinued the use of drugs for behavior modification. As a rule, patience and experience serve me well. In the rare case where drugs will be required, the use of a specialist in a hospital setting is advised.
Emergencies:
Emergencies will be seen on the same day if you call early. In the event of trauma, we will make every effort to get you in a quickly as possible.
Payments:
We accept cash, check, Visa and MasterCard, as well as, most major insurance and Medicaid. We will file your insurance on your behalf. You portion is payable at the time of the appointment.
Appointment Times:
When you make an appointment with Dr. Johnson, a block of time is set aside specifically for you. Consequently, you will be seen at your appointment time, without the usual “doctor office wait.” If you do not show up or show up late, the entire staff is left idle. PLEASE GIVE 24 HOURS NOTICE IF YOUR ARE UNABLE TO MAKE YOUR APPOINTMENT. We reserve the right to charge you if you are unable to make your appointment.
Dr. Johnson is so confident that you will be seen on time, he will give you the fee if he keeps you waiting an unreasonable length of time.

Please feel free to download the new patient registration HERE. After downloading it, you may fill out a printed version and bring it into the office with you or fill it out and email it to us. There is a button in the upper right hand corner “Submit Form” to email it directly from the form if you are using Adobe reader. Otherwise, save it and attach it to an email.
